About the role:

Based in our Burwood East office, this permanent part-time role is worked across three weekdays from 9:00am to 5:00pm, totalling 45 hours per fortnight.

As a Care Advisor, you will manage a caseload of older clients, partnering with them, their families and support networks to develop personalised care plans that promote wellbeing, independence and meaningful outcomes.

You'll play a vital role in coordinating services, conducting assessments and supporting clients through their aged care journey, including those participating in restorative care pathways, where clinical insight and goal-focused care planning can make a significant impact.

Partnering with health professionals, service providers and internal teams, you'll coordinate tailored supports that enable older people to live safely and independently at home while achieving meaningful, person-centred outcomes.

Key Responsibilities:

• Build strong, trusted relationships with clients and their families
• Conduct comprehensive assessments and identify individual goals and support needs
• Develop, implement and regularly review person-centred care plans
• Coordinate services and referrals to ensure holistic and seamless support
• Support clients to make informed choices regarding services and funding
• Monitor client wellbeing and proactively identify changing needs and risks
• Collaborate with nurses, allied health professionals, GPs and community providers
• Support restorative and wellness-focused approaches that maximise independence and function
• Manage a diverse caseload while maintaining accurate and compliant documentation
• Respond to client feedback, concerns and incidents professionally and empathetically

About you:

You are passionate about supporting older people to live independently, safely and with dignity in their own homes. You bring a person-centred approach to care, enjoy building meaningful relationships, and are confident working with clients, families and health professionals to achieve positive outcomes.

We're looking for someone who has:

• Experience working within the Aged Care, Community Services or Health sectors, with a strong understanding of client wellbeing, safety and quality care
• Demonstrated experience in the delivery of aged and community care services, supporting older people to achieve their goals and maintain independence
• Experience working within a clinical, nursing or allied health environment, with the ability to apply professional knowledge to client care planning and service coordination
• Case Management or Care Management experience, including the coordination of supports for clients with diverse and changing needs
• Experience conducting assessments, care planning, implementation, monitoring and evaluation of services, with accurate and timely documentation
• A strong commitment to person-centred care and Consumer Directed Care principles, ensuring clients remain at the centre of all decisions
• Excellent communication and relationship-building skills, with the ability to engage effectively with clients, carers, service providers, treating health professionals and the broader community
• The ability to develop and coordinate needs-based, consumer-focused services that support positive client outcomes
• A sound understanding of compliance requirements, including safety, privacy, equal opportunity and professional responsibilities within a care environment
• Strong organisational skills, the ability to manage competing priorities and maintain high-quality records
• The ability to work autonomously while thriving as part of a collaborative and supportive team

Essentials for working in Aged Care:

Applicants must have, or be willing to obtain:

• A current Victorian Driver Licence and access to a reliable, insured vehicle
• Current First Aid and CPR certification
• Working with Children Check (Employee)
• National Police Check
• Up-to-date vaccinations in accordance with Uniting AgeWell's Vaccination Policy
