Supporting our Home Care program based out of our Southern Tasmania Home Care and Community office in Rosny Park, the Community Registered Nurse services Uniting AgeWell clients in the suburbs of and surrounding Hobart, Tas.

• In-home clinical assessments, reviews and care needs for clients of Southern Home Care Program
• Provide clinical advice and support to clients and provide referrals to services where more specialized support/advice may be required
• Provide collaborative leadership, support and supervision to Care Advisors and Home Care staff
• Work closely with local allied health professionals to facilitate responsive care in our clients homes
• Provide health education and promotion, including social support groups and carer education 
• Promote a harmonious team approach by acting as a role model in coordinating daily routines and adhering to safe practice measures in medication management and achieving clinical support goals
• Full time opportunity, Monday to Friday during business hours
• Office based position with client visits - flexibility is negotiable with organisation and client needs

About you:

This is the place for you if you have:

• Bachelor of Nursing qualification with current AHPRA Div 1 Nursing registration (required)
• Minimum 3 years experience as a Registered Nurse in Community, Commonwealth Home Support or District Nursing
• Team player, flexible in your approach to work with a positive outlook 
• Endorsed for Continence Assessments and Prescription (highly desireable)
• Experienced in actively visiting clients in their homes
• Experienced with evaluating and implementing care support plans 
• Familiar with the My Aged Care portal 
• Proficiency in decision making, critical analysis and on the spot problem solving
• Demonstrated abilities to delegate and prioritise in a fast paced environment with exemplary communication skills in a team and one on one interactions
• Availability to work Monday to Friday full time
• Own your own registered and insured vehicle
• Thrive in a team and also appreciate the one to one interactions

• A current Working With Vulnerable People card
• Current and ongoing flu vaccinations
• Australian working rights
