About the role:

This position delivers a combination of centre-based and home-based services within the Northern Melbourne metro region. You will support clients aged 65+ accessing services through various funding streams, including Support at Home (SAH) and the Commonwealth Home Support Programme (CHSP). 
 
Your primary focus will be designing and implementing strengths-based, evidence-based interventions tailored to support individual independence and functional goals. 
 
To ensure your success, you will receive regular one-to-one supervision from a senior clinician. You will also be fully supported by a wider team of Exercise Physiologists, a dedicated Allied Health scheduling team, local on-site staff, and an Operations Manager. 
 
This is an ongoing permanent position. Full time or flexible FTE options are available, including family friendly hours and varied working days. 

 

About you:

We are seeking a motivated, client-centred clinician who aligns with our core values and is dedicated to continuous learning and best practice. The ideal candidate possesses: 
 
• Strong clinical and communication skills, with a dedication to evidence-based practice for older adults. 
• The ability to manage an individual caseload autonomously while collaborating effectively within a broader multidisciplinary team. 
• A proactive approach to seeking feedback and contributing to a positive, professional workplace culture. 

 

Role Requirements:

  • Recognised tertiary qualification in Exercise Physiology
  • Current ESSA accreditation
  •  Demonstrated experience in Community Aged Care, with working knowledge of HCP/SAH funding structures (desirable). 
  •  Comprehensive knowledge of evidence-based practice for older adults (aged 65+). 
  • Valid Australian Driver’s Licence and access to a reliable vehicle
  •  Full Australian working rights. 
  • Ability to successfully clear a National Police Check, NDIS Worker Screening Check, and Working with Children Check / Vulnerable People card. 
  • Current influenza vaccination.
