About the role:

Working with the Loddon Mallee North Support at Home Program, you will be based at our Swan Hill site. In this role, you will advocate for clients by facilitating discussions to identify their goals and develop tailored care plans. This is a part-time role working 60 hours per fortnight across 4 days per week.

Duties include:

  • Undertaking initial assessments of prospective clients and assist in making recommendations on capacity 
  • Assess, monitor, review, reassess the needs of clients within a holistic framework using a restorative and wellness approach 
  • Promote a harmonious team approach by acting as a mentor and supervisor to the Home Care Workers to ensure delivery of community services
  • Facilitate effective client pathway support through case discussions and case conferences, review of feedback including incidents, hazards and handovers and monitoring of quality indicators 
  • Conduct annual reviews with clients, including, revising their care plan, goals, services and budgets to support our clients in living life to fullest
  • Maintaining accurate client records, assessments, care plans, and documentation
  • Ensuring compliance with organisational, legislative, and program requirements

About you:

You are passionate about making a difference in the lives of older people, supporting their independence, wellbeing, and ability to make informed choices.

We're looking for someone with:

  • Experience within the aged care, health, community services, allied health, or disability sectors
  • Experience managing client caseloads and delivering person-centred, goal-focused support
  • Experience conducting assessments, developing care plans, coordinating services, and reviewing client outcomes
  • A strong understanding of Consumer Directed Care principles 
  • Excellent communication and relationship-building skills, with the ability to work collaboratively with clients, families, health professionals, service providers, and colleagues
  • The ability to manage competing priorities while maintaining accurate documentation and client records
  • A sound understanding of compliance, privacy, safety, and professional responsibilities within a care environment
  • The ability to work independently while contributing positively to a collaborative team environment
