About the role:
We have an exciting opportunity for a Full-Time 12 month fixed-term maternity leave cover role, working with our South Melbourne Metro team located in Berwick. The primary purpose of this position is to manage and to support the effective delivery of Uniting Agewell Commonwealth Home Support Programs (CHSP). This includes providing high quality support to clients, maintaining relationships with internal and external stakeholders, responding to client's enquiries, and supporting the client's journey to enhance their independence and quality of life.
Key Responsibilities:
- First point for enquiries and Intake of CHSP clients. Including managing of incoming referrals via My Aged Care Portal.
- Service coordination, assessment, and monitoring of clients’ needs and goals in order to ensure a client directed, timely, safe and high-quality service delivery.
- Develop and maintain positive stakeholder relationships and engagement and build networks to promote Uniting AgeWell services and advocate on behalf of clients.
- Conduct client follow up as required and liaise with schedulers and home care workers to ensure service delivery meets client expectations and needs.
- Complete all required documentation and actions to ensure quality standards are met.
- Apply a continuous improvement approach to Uniting AgeWell’s service delivery.
- Record all relevant client communication and information into the required client management systems.
About you:
You are passionate about what you do and want to make a difference in the lives of our clients to support them to live life to their full potential whilst remaining in their home safely.
- Experience in Aged Care Service, Community Services or Disability Services client coordination.
- Experience using client management systems
- Excellent communication skills with a compassionate and empathetic approach to your work.
- Ability to have ‘must have’ conversations.
- You will thrive in a fast paced and varied environment and have the ability to multi-task
- Strong attention to detail, and problem-solving abilities
- Excellent time management and highly organised
- Ability to work both within a team and autonomously
- Experience in assessing risks and developing management strategies to keep client safe.

- A current Working With Children Check card (Employee)
- Current and ongoing flu vaccinations
- Australian working rights
- Ability to pass a national police clearance and NDIS Clearance
