About the role:

The Clinical Nurse Consultant will utilize their extensive Nursing background and expertise in palliative care, playing a vital role in providing support for our nurses and carers in the field. With experience in supporting aged care services with end of life care coordination, the Clinical Nurse Consultant will lead care teams whilst providing oversight for clinical quality and risk, service performance and palliative care standards during end of life support at home. 
 
The Clinical Nurse Consultant influences and contributes to transformation of clinical services at Program and Site level, developing applying and promoting appropriate and innovative models of care.  The Clinical Nurse Consultant  acts as a clinical nursing resource and clinical advisor, to the specific care teams looking after Palliative care clients in their home or residential care setting.
 
In this role, you will:
  • Oversee and support the assessment, planning, implementation and evaluation of palliative care needs for customers in UA, to maximise quality outcomes
  • Assist in training, coaching & support of nurses and support workers, raising awareness of palliative care needs of customers and families
  • Partner with sites and services to support Palliative care needs, to ensure that care is systematically planned, implemented and evaluated and that approaches to palliative care support choices and are consistent with current best practice
  • Liaising with external organisations and palliative care consortiums in support of palliative care needs
  • To assist people diagnosed with palliative conditions, their carers and family members in accessing practical in-home supports particularly in the end stages of life. The program aims to achieve the best possible service outcomes, reduce stress, improve quality of life and allow people to choose home as their preferred setting for end of life care or assist them to remain at home for as long as practically possible

  • AHPRA Registration
  • Minimum of 3 years’ experience providing palliative care services
  • Experience in providing palliative care in community setting is highly regarded
  • Experience in Clinical Care Coordinator role is highly regarded
  • Excellent analytical problem-solving skills
  • Exceptional customer service and clinical care
  •  Excellent interpersonal and communication skills with a compassionate and empathetic approach to your work and attention to       detail
  •  The ability to build relationships with customers, their family members and work closely with stakeholders including residential, home based (clinical) palliative care specialists and hospitals
  •  Strong case management and stakeholder engagement skills and demonstrated knowledge of palliative service system
  • Excellent written and verbal communication skills
  • Highly developed organisation, analytical and problem-solving skills
  • Thrive in a team and also appreciate the one to one interactions

  • A current Working With Children Check card / Vulnerable People card
  • Current and ongoing flu vaccinations
  • Australian working rights
