About the role:

The Support-at-Home Manager (Home Care Program Manager) is a senior leadership role within Uniting AgeWell’s Home Care division, responsible for leading the delivery of high quality, compliant and consumer‑directed Home Care and Respite services across Swan Hill and surrounding communities.
 
Working closely with the Director, you will provide operational leadership to multidisciplinary teams, ensuring services are safe, sustainable, and aligned to clinical governance and aged care standards. You will balance people leadership, service quality, and financial accountability while driving continuous improvement and responding to the evolving aged care environment.

In this role, you will:

  • Lead and support multidisciplinary Home Care teams across Victoria and Tasmania, providing clear direction, coaching, and support to enable staff to deliver high‑quality, consumer‑directed care outcomes.
  • Deliver safe, compliant and person‑centred services, ensuring assessment, care planning, service delivery and review processes meet legislative requirements, clinical governance expectations, and Uniting AgeWell standards.
  • Drive workforce capability, engagement and performance through effective leadership, roster oversight, role clarity, development opportunities, and by fostering a cohesive, values‑led team culture.
  • Manage programs within agreed budgets and resource frameworks, monitoring financial performance, identifying risks and opportunities, and balancing service demand with sustainable workforce and cost outcomes.
  • Embed quality improvement and risk management practices, supporting teams to understand and comply with program standards, participate in audits, respond to feedback, and continuously improve service delivery.
  • Build and maintain strong relationships with clients, families, carers, internal teams, and external partners to ensure coordinated services, positive customer experiences, and effective collaboration across the care continuum.
  • This is a full time role working Monday-Friday

  • Strong people and operational leadership
  • Budget and service delivery experience
  • Knowledge of Support-at-Home Care programs and standards
  • Tertiary qualification or equivalent experience

You will need:

  • A current Working With Children’s Check card
  • Full Australian working rights
  • Current police record check or NDIS(or be willing to undertake this process)
  • Current and ongoing flu vaccinations
