About the role:
In this role, you will:
- Lead and support multidisciplinary Home Care teams across Victoria and Tasmania, providing clear direction, coaching, and support to enable staff to deliver high‑quality, consumer‑directed care outcomes.
- Deliver safe, compliant and person‑centred services, ensuring assessment, care planning, service delivery and review processes meet legislative requirements, clinical governance expectations, and Uniting AgeWell standards.
- Drive workforce capability, engagement and performance through effective leadership, roster oversight, role clarity, development opportunities, and by fostering a cohesive, values‑led team culture.
- Manage programs within agreed budgets and resource frameworks, monitoring financial performance, identifying risks and opportunities, and balancing service demand with sustainable workforce and cost outcomes.
- Embed quality improvement and risk management practices, supporting teams to understand and comply with program standards, participate in audits, respond to feedback, and continuously improve service delivery.
- Build and maintain strong relationships with clients, families, carers, internal teams, and external partners to ensure coordinated services, positive customer experiences, and effective collaboration across the care continuum.
- This is a full time role working Monday-Friday

- Demonstratable Leadership experience in aged care or health
- Strong people and operational leadership
- Budget and service delivery experience
- Knowledge of Support-at-Home Care programs and standards
- Tertiary qualification or equivalent experience

You will need:
- A current Working With Children’s Check card
- Full Australian working rights
- Current police record check or NDIS(or be willing to undertake this process)
- Current and ongoing flu vaccinations
