About the role:
We are delighted to offer an exciting permanent full‑time opportunity for a Care Advisor to join our Support at Home Intake Team, based at our Braybrook site. This role is full time, working Monday to Friday, 9:00am – 5:00pm
This position is ideal for someone who is passionate about supporting older people at a pivotal moment in their aged care journey, providing guidance, reassurance and practical support when it matters most.
As an Intake Care Advisor, you will be a trusted first point of contact for clients newly approved or allocated a Support at Home package, guiding them through the intake and onboarding process with empathy, clarity, and professionalism.
Working collaboratively with our West Metro and North Metro teams, you will help ensure every client experiences a smooth, welcoming, and well‑coordinated transition into services—feeling informed, confident, and genuinely supported from day one.
In this rewarding role, you will:
- Provide end‑to‑end intake support for clients newly allocated Support at Home funding.
- Conduct initial meetings with clients and/or their representatives to:
- Develop personalised, goal‑focused care plans aligned to individual needs and preferences
- Clearly explain Support at Home packages and funding
- Discuss available services and support options
- Outline next steps and timeframes with confidence and reassurance
- Guide clients through the onboarding process, including required documentation and assessments.
- Work closely with internal teams to coordinate the timely and effective commencement of services.
- Maintain accurate and compliant client records in line with aged care legislation and organisational requirements.
- Deliver exceptional customer service and build strong, respectful relationships with clients, families, and key stakeholders—ensuring a positive and reassuring experience with Uniting AgeWell.
About You
To succeed in this role, you will bring:
- Proven experience in intake, assessment, or client onboarding within aged care or community services.
- A strong understanding of the community aged care system and client pathways.
- Demonstrated experience working within the Support at Home program.
- Excellent interpersonal and communication skills, with the ability to engage sensitively with a diverse client group.
- Strong organisational skills, with the ability to manage competing priorities and work autonomously.
- A high level of attention to detail, with confidence in documentation and data entry
