About the role:

Working out of our office in Braybrook you will play an important role in coordinating Commonwealth Home Support Programme (CHSP) services that help older people remain safe, independent and connected to their communities. You will support client intake, service planning and reviews, coordinate service delivery, respond to changing client needs, and work closely with clients, families, and support staff to deliver high-quality, person-centred care while maintaining accurate documentation and compliance with CHSP requirements.

You will be the key contact for clients and play an active role in the coordination of the client’s care needs.  

This is a full-time, office-based role with standard working hours of Monday to Friday, 9:00am to 5:00pm. The successful candidate will be required to work onsite each day.

Tasks include: 

  • Conducting client intake via referrals through My Aged Care
  • Completion of service planning and reviews, maintaining accurate care plan documents and remaining responsive to changing care needs
  • Maintaining accurate client records and service documentation
  • Responding to client enquiries and providing exceptional customer service
  • Monitoring service delivery and escalating where appropriate
  • Adherence to CHSP compliance requirements
  • Working collaboratively within a multi-disciplinary team to deliver high-quality, person-centred care

About you:

To be successful in this role, you will have:

  • Experience working within the Commonwealth Home Support Program
  • A sound understanding of the Aged Care Quality Standards
  • Strong customer service skills and the ability to build trusted relationships with clients, families and colleagues
  • Excellent communication skills, with the ability to discuss changing client needs and coordinate appropriate services
  • Strong organisational and time management skills, with the ability to manage competing priorities
  • High attention to detail and accuracy when maintaining client records and documentation
  • Confidence using computer systems, databases and Microsoft Office applications
  • Thrive in a team and also appreciate the one to one interactions

  • A current Working With Children Check
  • Current and ongoing flu vaccinations
  • Australian working rights
  • Ability to pass a national police clearance and NDIS Clearance
