About the role:

Supporting our Home Care and Community program based out of our Berwick office, the Community Registered Nurse services Uniting AgeWell clients in the South Melbourne suburbs of the Mornington Peninsula, including Mornington and surrounding suburbs through to the Frankston area

  • In-home clinical assessments, reviews and care needs for clients in the South Melbourne suburbs
  • Provide clinical care including Wound Care, Palliative Care, Catheter Care and Incontinence support
  • Provide clinical advice and support to clients and provide referrals to services where more specialized support/advice may be required
  • Provide collaborative leadership, support and supervision to Care Advisors and Home Care staff
  • Work closely with local allied health professionals to facilitate responsive care in our clients homes
  • Provide health education and promotion, including social support groups, clients and families as well as carer education 
  • Promote a harmonious team approach by acting as a role model in coordinating daily routines and adhering to safe practice measures in medication management and achieving clinical support goals
  • Permanent Full time opportunity, Monday to Friday during business hours (eg 8 or 8.30am start)
  • Client focused position with 1 - 2 days of administration in the Berwick office (flexibility is negotiable with organisation and client needs)

About you:

This is the place for you if you have:

  • Bachelor of Nursing or equivalent qualification with current AHPRA Div 1 Nursing registration (required)
  • Minimum 3 years experience as a Registered Nurse in Community, Commonwealth Home Support or District Nursing (required)
  • Team player, flexible in your approach to work with a positive outlook 
  • Demonstrable strong clinical experience with wound care, palliative care, continence aids, catheter management and person centered care
  • Endorsed for Continence Assessments and Prescription (highly desirable)
  • Experienced in actively visiting clients in their homes
  • Experienced with evaluating and implementing care support plans 
  • Familiar with the My Aged Care portal and Alayacare (or have a proficient IT mindset to learn quick and on the go)
  • Proficiency in decision making, critical analysis and on the spot problem solving
  • Demonstrated abilities to delegate and prioritise in a fast paced environment with exemplary communication skills in a team and one on one interactions
  • Availability to work Monday to Friday full time 
  • Own your own registered and insured vehicle
