About the role
Based from our Swan Hill office, the Community Registered Nurse supports Uniting AgeWell clients across Swan Hill, Kerang, and surrounding regions as part of our Support at Home program.
In this role, you will:

Conduct in-home clinical assessments, reviews, and ongoing evaluation of client care needs

Provide clinical advice and support to clients, including referrals to appropriate services where specialised input is required
-
Deliver collaborative leadership, guidance, and clinical supervision to Care Advisors and Home Care staff
-
Work closely with local allied health professionals to ensure timely, coordinated, and responsive care in clients’ homes
-
Provide health education and health promotion initiatives, including carer education and social support groups
-
Promote a positive and collaborative team environment by role modelling best practice, supporting daily care routines, ensuring safe medication management, and contributing to the achievement of clinical care goals
This is a permanent full-time position, working Monday to Friday during business hours.
About you
This role is ideal for you if you have:

A Bachelor of Nursing or equivalent qualification with current AHPRA Division 1 registration (required)

A minimum of 3–5 years’ experience as a Registered Nurse within community health, Commonwealth Home Support, or district nursing settings (required)

A collaborative, flexible approach to work, with a positive and solution-focused mindset
-
Endorsement for continence assessments and prescription (highly desirable)
-
Demonstrated experience visiting clients in their homes and delivering community-based care
-
Experience in evaluating, implementing, and reviewing care and support plans
-
Working knowledge of the My Aged Care portal

A genuinely kind and caring nature, with a strong commitment to person-centred care and client advocacy

Strong decision-making, critical thinking, and on-the-spot problem-solving skills
-
Proven ability to delegate and prioritise effectively in a fast-paced environment, supported by clear and confident communication skills

Availability to work full-time, Monday to Friday

A current Victorian Working With Children Check (Employee)
