About the role:
We are recruiting two Team Leaders for our home care program in Mildura, each with a distinct operational focus outlined below.
Home Care Team Leader Operations (Care Organisers and Administration)
This role leads the Care Organisers who coordinate and schedule the home care workforce for customer visits. You will oversee daily rostering, manage unplanned leave and service disruptions, and ensure services are delivered efficiently, compliantly and in line with customer needs and preferences.
Home Care Team Leader Operations (Care Advisors / Care Planning)
This role leads the Care Advisors who support customers through care planning, coordination and review. You will ensure care plans are developed, implemented and monitored in line with funding requirements, quality standards and Uniting AgeWell values.
This is a Full Time role working Monday - Friday, 9am - 5pm.
Key Responsibilities:
- Lead and supervise your assigned team to deliver consistent, high quality services
- Ensure daily operations run smoothly, including scheduling or care planning activities
- Monitor performance, compliance and funded program KPIs
- Coach, mentor and support staff performance and development
- Manage risks, incidents and service disruptions appropriately
- Work collaboratively with leaders, teams and stakeholders across Home Care
About you:
This is the place for you if you have:
- Experience in Managing teams within an Age Care sitting (desirable)
- Extensive experience in a community aged care setting
- Previous care planning and/or rostering/scheduling experience in community services (CHSP/Support-at-Home)
- Ability to manage multiple requests in a fast-paced environment
- Experience within the aged care sector
- Thrive in a team and also appreciate the one to one interactions

Essentials for working in Aged Care:
- A current Working With Children Check card
- Current and ongoing flu vaccinations
- Australian working rights
- Ability to pass a national police clearance and NDIS Clearance
