About the role:

As a Care Advisor (Case Manager) with our North West Metro Melbourne and Barwon Home Care Program, you’ll jump into a fast‑paced, purpose-driven role where you can make a real difference every single day. You’ll take the lead on your own caseload of clients, championing their independence and empowering them to continue living confidently and comfortably in their own homes.

You’ll work from either our Northern or Western office, depending on the successful applicant, joining a vibrant, supportive team that’s passionate about positive outcomes and community impact.

This is a full-time role, Monday to Friday, 9am to 5pm, giving you the perfect balance of stability, teamwork and truly meaningful work.

Key elements of the role include: 

  • Performing initial assessments of prospective clients, identifying core needs and developing care plans that align with the participants assessed need
  • Managing budgets and ensuring services are delivered within financial constraints
  • Working closely with Care Organisers to coordinate the delivery of services as outlined in the care plan
  • Liaising with referring healthcare providers, allied health professionals, social workers, and community services
  • Facilitating effective client pathway support through case discussions, case conferences, and feedback reviews, including incident management and complaint resolution
  • Maintaining accurate and up-to-date records of client assessments, care plans, services provided, and any changes or incidents.
  • Coordinating services within business guidelines
  • Ensuring all service delivery meets the Aged Care Quality Standards and maintains accurate records for program audits

About You

You are a supportive, friendly, and highly organised individual with strong time‑management, leadership and customer service skills. You thrive in a dynamic, innovative environment and love being part of a skilled, committed team that genuinely believes in the quality of care they deliver. You bring a positive attitude, a collaborative spirit, and a passion for making a difference. You also have:
  • Demonstrated skills and experience in Case Management
  • Experience and expertise in the coordination and provision of Aged and Community services; Support at Home experience highly desirable
  • Experience and expertise in working in a Clinical or Allied Health setting
  • A genuine desire to deliver outstanding customer service in all stakeholder interactions
  • Strong communication and interpersonal skills with a focus on building and fostering meaningful relationships
  • Are committed to being part of a workplace culture based on the BESway of life (Believe, Excel, Support and Trust)

Essentials for working in Aged Care:

  • A current Working With Children or willingness to obtain one
  • Current and ongoing flu vaccinations
  • Australian working rights
  • Ability to pass a national police clearance
