About the role

Working within the Loddon Mallee North Support at Home Program, you will be based in our Mildura office and play a key role in supporting older people to live independently in their own homes.

As a Care Advisor, you will manage a caseload of clients, working closely with them, their families and support networks to identify goals, develop personalised care plans and coordinate services that promote wellbeing, choice and independence.

This is a full-time position working Monday to Friday.

Duties may include:

  • Building trusted relationships with clients and their families through regular contact and support
  • Conducting assessments and developing personalised care plans that promote independence, wellbeing and choice
  • Reviewing and adjusting care plans as client needs change
  • Coordinating services and referrals to ensure seamless, client-focused care
  • Supporting clients to make informed decisions about their services, goals and budgets
  • Monitoring client wellbeing and identifying emerging risks or support needs
  • Working closely with health professionals and community services to achieve positive outcomes
  • Managing a diverse caseload while maintaining accurate records and documentation
  • Responding to client feedback, incidents and concerns with professionalism and empathy
  • Delivering exceptional customer service while supporting older people to live independently at home

About you

You are passionate about supporting older people to live independently and achieve their goals. You enjoy building meaningful relationships, navigating complex situations and delivering high-quality, person-centred support.

We're looking for someone with:

  • Experience managing client caseloads and delivering person-centred, goal-focused support within the aged care, health, community services, allied health or disability sectors
  • Experience conducting assessments, developing care plans, coordinating services and reviewing client outcomes
  • A strong understanding of Consumer Directed Care principles and person-centred practice
  • Excellent communication and relationship-building skills, with the ability to work collaboratively with clients, families, health professionals and service providers
  • Strong organisational skills and the ability to manage competing priorities while maintaining accurate records and documentation
  • A sound understanding of compliance, privacy, safety and professional responsibilities within a care environment
  • The ability to work independently while contributing positively to a collaborative team environment
  • A commitment to our BEST philosophy of Believe, Excel, Support and Trust

Essential Requirements

You must have, or be willing to obtain:

  • Current First Aid and CPR certificates
  • A Victorian Driver Licence
  • Access to a reliable, registered and insured vehicle for work-related travel
  • A Victorian Working With Children Check (Employee)
  • Current and ongoing flu vaccination
  • Australian working rights
  • A National Police Check
