About the role:

As a Care Advisor in Home Care, you’ll be the key contact for clients, guiding them through their care journey. You’ll work directly with older people and their families to understand goals, coordinate services and ensure care is tailored, practical and within budget. This role brings together case management, stakeholder engagement and service coordination to deliver strong client outcomes. You’ll operate within a multidisciplinary team, balancing quality care with funding and compliance requirements. It’s a role where your decisions have a direct and visible impact on people’s independence and wellbeing
  • Lead client assessments and develop goal-directed care plans aligned to individual needs
  • Coordinate services across internal teams and external providers to deliver outcomes
  • Monitor and manage client budgets, ensuring services remain sustainable and effective
  • Build strong relationships with clients, families and healthcare professionals
  • Support client intake and program growth through proactive community engagement
  • Ensure documentation, reporting and compliance meet aged care standards
  • Advocate for clients, connecting them with services that support independence and quality of life
This is a permanent role, working Monday to Friday based out of our Ballarat office.

About you:

  • Proven experience in aged care, health or community services delivery
  • Strong case management capability with measurable client outcomes
  • Experience building and managing care plans aligned to funding frameworks
  • Ability to manage budgets and balance quality care with constraints
  • Confident stakeholder engagement across clients, families and providers
  • Experience working within clinical or allied health settings
  • Sound understanding of aged care standards and compliance requirements
  • Ability to manage multiple clients and priorities in a community setting

  • A current Working With Children Check card
