About the Role
As a Care Advisor (Case Manager) supporting our North Melbourne Metro Home Care Program, you will be responsible for managing a caseload of clients, playing a vital role in advocating for them to remain living independently in their own homes. The position will be based a our office in Northcote.
Key elements of the role include
- Performing initial assessments of prospective clients, identifying core needs and developing care plans that align with the participants assessed need
- Managing budgets and ensuring services are delivered within financial constraints
- Working closely with Care Organisers to coordinate the delivery of services as outlined in the care plan
- Liaising with referring healthcare providers, allied health professionals, social workers, and community services
- Facilitating effective client pathway support through case discussions, case conferences, and feedback reviews, including incident management and complaint resolution
- Maintaining accurate and up-to-date records of client assessments, care plans, services provided, and any changes or incidents.
- Coordinating services within business guidelines
- Ensuring all service delivery meets the Aged Care Quality Standards and maintains accurate records for program audits
About You
You are a supportive, friendly, and organised individual with excellent time management, leadership and customer services skills. You thrive in a dynamic and innovative environment and enjoy being part of a highly skilled team who believe in the care and service that they provide.
You also have:
- Demonstrated skills and experience in Case Management

AHPRA registration (highly desirable).
- Experience and expertise in the coordination and provision of Aged and Community services; Support at Home experience highly desirable
- Experience and expertise in working in a Clinical or Allied Health setting
- A genuine desire to deliver outstanding customer service in all stakeholder interactions
- Strong communication and interpersonal skills with a focus on building and fostering meaningful relationships

Essentials for working in Aged Care:
- A current Working With Children or willingness to obtain one
