About the role:

You will deliver centre‑based and home‑based Physiotherapy services in the West Melbourne metro region for clients aged 65+ across multiple funding streams, including Support at Home (SAH) and CHSP. The role focuses on strengths‑based, goal‑directed interventions that support independence. Travel requirements may vary based on service demand and client need across the region.
 
This is an ongoing permanent position. Full‑time or flexible FTE options are available, including family‑friendly hours. You will receive regular one‑to‑one supervision from the Practice Lead and be supported by a team of physiotherapists, the Allied Health scheduling team, the Operations Manager, and local on‑site staff.

About You:

You are a physiotherapist with strong clinical skills and a client‑centred approach. You work to evidence‑based standards, manage change well, and contribute positively to team culture. You seek feedback, value learning, and are committed to supporting older adults to achieve their goals.

This is the place for you if you:

  • Current registration with AHPRA
  • Recognised tertiary qualification in Physiotherapy
  • Experience in Community Aged Care with HCP/SAH clients
  • Knowledge of practice with clients aged 65+
  • Ability to manage an individual caseload independently
  • Have full Australian working rights and are able to pass a national police clearance
  • Thrive in a team and also appreciate the one to one interactions
  •  Ability to collaborate within a multidisciplinary team
  • A valid driver's license and access to a car

  • A current Working With Children Check card / Vulnerable People card
  • Current and ongoing flu vaccinations
  • Australian working rights
  • Ability to pass a national police clearance and NDIS Clearance
