About the Role
 
Our Home Care Programs support older people to remain independent in their own homes for as long as possible. We deliver personalised aged care services through both the Support at Home and Commonwealth Home Support Programme (CHSP) across the Hume and Grampians regions.

We are seeking a Care Organiser to join our team, based in either our Ballarat or Wangaratta office. In this role, you will coordinate and schedule services across both programs, ensuring clients receive the right support at the right time. Working closely with clients, Care Advisors, support staff and service providers, you will manage service schedules, respond to changing client needs and help deliver high-quality, person-centred care.

This is a full-time position working Monday to Friday.

Duties may include:

  • Coordinating and scheduling services across the Support at Home and Commonwealth Home Support Programme (CHSP)
  • Matching care staff to client needs, preferences and availability
  • Managing service schedules and responding to changes, cancellations and urgent requests
  • Liaising with clients, families, Care Advisors and support staff to ensure continuity of care
  • Monitoring service delivery and making scheduling adjustments to meet client needs
  • Maintaining accurate client records and scheduling information
  • Responding to client and staff enquiries and providing exceptional customer service
  • Ensuring scheduling and administrative processes comply with organisational policies and funding requirements
  • Working collaboratively with the broader Home Care team to support efficient service delivery and positive client outcomes

About You

This role is ideally suited to someone with experience coordinating and scheduling services within the aged care, health or community services sector. You enjoy solving problems, managing competing priorities and ensuring the right people are in the right place at the right time to deliver high-quality care.

To be successful in this role, you will have:

  • Experience coordinating, scheduling or rostering services within aged care, health or community services
  • Strong organisational and time management skills, with the ability to manage competing priorities
  • Excellent communication and customer service skills, with the ability to build positive relationships with clients, families and colleagues
  • A proactive, solutions-focused approach with the ability to adapt to changing priorities
  • High attention to detail and accuracy when maintaining schedules and client records
  • Confidence using computer systems, databases and Microsoft Office applications
  • A sound understanding of the Support at Home Program, Commonwealth Home Support Programme (CHSP) or the broader community aged care sector (desirable)

You must have, or be willing to obtain:

  • A Victorian Working With Children Check (Employee)
  • Current and ongoing flu vaccination
  • A National Police Check
  • Australian working rights
