About the Role
 
Our Hume Home Care Program supports older people to remain independent in their own homes for as long as possible. We deliver personalised aged care services to clients throughout the Hume region and are currently seeking a Care Organiser to join our team.

Based in our Wangaratta office, you will play an important role in coordinating Commonwealth Home Support Programme (CHSP) services that help older people remain safe, independent and connected to their communities. You will support service planning and reviews, coordinate service delivery, respond to changing client needs, and work closely with clients, families, Care Advisors and support staff to deliver high-quality, person-centred care while maintaining accurate documentation and compliance with CHSP requirements.

This is a full-time position working Monday to Friday.

Duties may include:

  • Coordinating Commonwealth Home Support Programme (CHSP) services to meet clients' individual needs
  • Conducting client service planning and reviews, ensuring services remain responsive to changing needs
  • Coordinating service changes and liaising with clients, families and support staff
  • Maintaining accurate client records and service documentation
  • Responding to client enquiries and providing exceptional customer service
  • Monitoring service delivery and escalating concerns where appropriate
  • Ensuring documentation and administrative processes comply with CHSP requirements
  • Working collaboratively with Care Advisors and the broader Home Care team to deliver high-quality, person-centred care

About You

You are organised, approachable and genuinely enjoy helping people. You thrive in a busy environment, enjoy building positive relationships and take pride in delivering exceptional customer service.

This role is ideally suited to someone with experience coordinating aged care services, conducting client service planning and reviews, or supporting the delivery of community-based aged care programs. You are confident working with clients and families to understand changing needs, coordinate appropriate services and ensure accurate documentation and compliance.

To be successful in this role, you will have:

  • Experience coordinating aged care services, including client service planning and reviews
  • A sound understanding of the Commonwealth Home Support Programme (CHSP) or broader community aged care sector (desirable)
  • Strong customer service skills and the ability to build trusted relationships with clients, families and colleagues
  • Excellent communication skills, with the ability to discuss changing client needs and coordinate appropriate services
  • Strong organisational and time management skills, with the ability to manage competing priorities
  • High attention to detail and accuracy when maintaining client records and documentation
  • Confidence using computer systems, databases and Microsoft Office applications

Essential Requirements

You must have, or be willing to obtain:

  • A Victorian Working With Children Check (Employee)
  • Current and ongoing flu vaccination
  • A National Police Check
  • Australian working rights
